Some students are selected for verification, at random, to confirm that the data reported on your FAFSA form is accurate. Students will be asked to provide selected documentation to our office, so that we may review it for accuracy. If you are selected, please ensure that you submit the requested documentation in a timely manner to avoid delays in awarding.

Please print out the form you need below and fill it out completely, including a "wet" signature (aka print it out and physically sign it) or complete electronically, when available.
