Loans accepted / requested after deadline dates might not be processed and paid. With the exceptions of cancellations, loan requests will not be processed until after the student has been offered, and accepts the initial loan offer. Please allow up to 10 business days for processing.

Completing the correct Financial Aid Form is essential to ensuring your request is processed. Whether you are requesting a loan adjustment, submitting a tax filing status, or anything in between, it starts with selecting the correct form and reading the instructions carefully.

Steps to Complete Your Forms 

  1. Please print out the form you need below and fill it out completely, including a "wet" signature (aka print it out and physically sign it) or complete electronically, when available. 

  2. You can email your form to financialaid@pikespeak.edu or turn it in at any of our Enrollment Services Centers or fax to (719) 502-2074.

  3. If you need any other forms or have any questions, please contact Financial Aid (719) 502-3000.

***Forms must be completed in blue or black ink. Failure to accurately complete these forms may result in a delay of processing or change of financial aid eligibility. Additional documentation may be requested. Read instructions carefully before completing.

Loan request forms are available for eligible students who wish to request an increase in their student loans. We encourage you to carefully consider your expenses before seeking additional loan funding. Please note that you must have already completed and submitted the Free Application for Federal Student Aid (FAFSA) to apply for a loan.

If you have taken out a student loan and wish to cancel it, please complete this form. You will receive a notification at your student email once your awards are updated. Use this form to CANCEL AN ENTIRE LOAN for one or more semesters. To reduce your loan amount, use the Loan Reduction in the forms section below.

We strongly encourage you to complete this form in person with the Financial Aid Office. If you are outside the greater Colorado Springs area and need to mail it, please provide all necessary documentation. A notary must complete their section on the form, and you must include a copy of your unexpired government-issued ID.

What is “family size”? Family size replaces the term “household size” on the latest FAFSA form. It captures the appropriate number of family members and dependents in the applicant’s household. For tax-filers, family size will be automatically calculated based on the number of individuals claimed as exemptions on the tax return.

Important Notarization Information

Certain forms must be completed in person at the PPSC Enrollment Center or in front of a Notary. These specific forms will provide notarization details below. They must be completed either:

1. In Person at PPSC
            -OR-
2. In Front of a Notary*
    • The Notary must make a copy of your ID.
    • On the copy of your ID, the Notary must write the following:
      • "Copy Made from Original"
      • Their Name
      • The Date of Notarization
    • You can then mailed the notarized forms to PPSC at the below address:

Requesting Tax Documents

Below are the options to obtain tax documentation required for FAFSA verification.

Start by visiting the IRS website

  1. Request Online Account: Create an account with the IRS or ID.me to download your transcripts
    • The Record of Account Transcript, Return Transcript, and Wage & Income Transcript should be downloaded as this includes all the information we need.
    • Note: You will need a ID, drivers license, credit card, student loan, mortgage, home equity loan, or auto loan to verify your identity to create an account. If you do not have one of these, you will need to request the transcript by mail or through Form 4506-T.
  2. Request by Mail: If you still live at the address listed on your 2022/2023 tax return, click “Get Transcript by Mail” and complete two requests: one for the Account Transcript and one for the Return Transcript for 2022/23. The IRS will mail the transcripts to the address listed on the 2022/23 tax return.
    • Note: The IRS does not have an option to request a Record of Account Transcript through the online mail request system.
  3. Recently Moved or Changed Addresses? If you no longer have access to mail at the address on your 2022/23 tax return, use Form 4506-T from the IRS to request a Record of Account Transcript (check box 6c) for the year that ended December 31, 2022/23 (line 9). For verification on Non-Filing, please mark box 7 and 8, again for the year ending December 31, 2022/23. Mail or fax Form 4506-T to the address listed on Page 2.

Your FAFSA Submission Summary may say that you’ve been Lightbulb Iconselected for verification. This process confirms that your FAFSA is accurate. If selected, PPSC will request additional documentation to support your information. Learn more by visiting StudentAid.gov.
